The 835 file is a formatted report created by a payer to detail how claims and medical procedures were adjudicated. The 835 is the industry standard across the country for reporting healthcare billing details.
These files contain critical payment information, but their complex EDI format makes manual analysis time-consuming and error-prone. These 835 files are a goldmine of information that you should be leveraging.

835 Analytics
835 analysis is the process of extracting, organizing, and interpreting payment data from 835 EDI files to understand financial performance, identify denial patterns, and improve revenue cycle management. Whether you’re tracking payment trends, investigating underpayments, or monitoring denial rates, effective 835 analysis is essential for optimizing your revenue cycle.
This guide will walk you through everything you need to know about analyzing 835 files, from understanding what’s inside them to implementing a systematic analysis process that reveals actionable insights.
The goal is to transform raw EDI data into actionable business intelligence that helps practices make data-driven decisions about revenue cycle operations. Should you invest in training for front office, or medical billing teams, negotiate better contractual rates with certain payers or institute eligibility checks? Do you have population of claims needing to be reworked or resubmitted?
Most healthcare providers receive 835 files through their clearinghouse, payer portal, or direct EDI connections. Without proper 835 analysis, practices miss critical opportunities to identify payment issues, understand denial trends, and improve financial performance.
A dedicated 835 analysis tool like EMS Insights will automatically parses 835 files, extracts data, and present it in an easy-to-understand format. Even if you already have healthcare billing analytics software, it’s worth getting a second opinion. EMS Insights will have large cost savings compared to traditional business intelligence solutions like Tableau or Power BI with none of the data warehouse or knowledge requirements. Even if you just want to convert EDI to Excel, EMS Insights can accomplish this for you.

Identify Payment Discrepancies
835 analysis reveals when payments don’t match expected amounts. This can occur with a denial, which is when nothing is paid for a particular claim or an adjustment, which is a claim underpayment.
Adjustments, especially contractual adjustments, are normal. Unexpected underpayments, missing payments, or incorrect adjustments need investigation. Regular analysis helps catch these issues before they become costly problems. Using an industry leading 835 analysis tool like EMS Insights can help you work through these payment discrepancies.

An adjustment lives in the CAS segment on an 835 005010X221 file. These adjustments have a few important parts:
- Adjustment group code: CO = Contractual Obligation, PR = Patient Responsibility, OA = Other Adjustment and PI = Payor Initiated. This is the grouping this adjustment falls into.
- Adjustment reason code: aka CARC code Claim Adjustment Reason Codes, RARC code Remittance Advice Remark Codes. This is a code explaining why adjustments occurred and providing additional context.
- Adjustment amounts: Each adjustment comes with an amount. The sum of all adjustments for the claim should equal the difference between what was charged and what was paid.
835 files can also have a Provider-Level Adjustment. These are stored in the PLB Segment of the 835. These are adjustments that apply to the entire payment and can be for reasons such as offsets from previous payments, recoupments or overpayments.

Understanding Denial Patterns
Denial rates vary by payer, procedure code, and provider. Reviewing the 835 data identifies which denial codes (CARC codes) appear most frequently and which ones have the highest financial impact. This helps prioritize denial management efforts and target the most problematic areas. Also seeing the trend of these adjustment codes over time can help you understand what has been helping. Do I actually see the adjustment amount going down month over month or week over week?
